Beyond “We’ve Always Done It This Way”: Rethinking Legacy Foil in Medical Device Packaging

For decades, foil has been the default solution for high-barrier medical device packaging. When a product required protection from moisture, oxygen, light, or environmental exposure, the answer was often simple: add a foil layer.

In conversations with medical device manufacturers, packaging engineers, and supply chain leaders, I continue to hear a familiar phrase:

“We’ve always done it this way.”

There is value in proven solutions. Packaging decisions are built on years of validation, regulatory requirements, and real-world performance. However, relying on what has always worked can make it easy to overlook whether a legacy approach still aligns with today’s priorities.

The question is not whether foil has been successful — it has. The question is whether it remains the best solution as medical device companies face new demands around sustainability, operational efficiency, supply chain flexibility, and next-generation packaging performance.

Today, more organizations are taking a closer look at their packaging strategies and challenging long-standing assumptions. Not because foil has failed, but because the needs of the industry have evolved.

The Cost of Familiarity

Most packaging choices are made for solid reasons. A foil structure approved ten years ago probably met the barrier needs back then. It passed tests, helped with regulatory submissions, and became part of the manufacturing routine.

The problem is that packaging systems are rarely reviewed unless something goes wrong. At the same time, expectations keep changing.

Regulatory scrutiny is rising. Supply chains are less stable. Usability and human factors are getting more focus. Sustainability is affecting buying choices. Healthcare providers also want packaging that is easier to check and use.

Supply Chain Vulnerability Is a Packaging Risk

In recent years, we’ve seen how fragile global supply chains can be. Material shortages, long lead times, shipping problems, and global uncertainty have made manufacturers rethink their sourcing strategies.

Foil-based laminations often need special materials, take longer to produce, and rely on fewer suppliers compared to many other options.

When key packaging materials are hard to get, the impact goes beyond just buying them. Production can be delayed, inventory costs can go up, validation gets harder, and product launches might be pushed back.

People often see packaging as something that happens later in the process, but it’s now becoming a key part of supply chain strategy.

If You Can’t See It, Can You Trust It?

One big drawback of foil packaging is that you can’t see through it. Traditional foil creates a solid barrier, so you can’t visually check the device inside. This might not seem important until someone needs to confirm the product’s position, parts, or package condition before opening.

In many healthcare settings, being able to see the product is becoming more important. It helps reduce mistakes, makes work more efficient, and supports better quality checks.

Clear high-barrier packaging is changing things because it lets users check the contents without losing protection. This not only makes packaging easier to use but can also cut down on handling mistakes and boost confidence when using the product.

High Barrier No Longer Means Foil

In the past, foil provided a level of barrier protection that other materials just couldn’t match. But now, that difference has gotten much smaller.

Thanks to advances in polymer science, coatings, and multilayer designs, we now have clear materials that offer strong protection against moisture and oxygen while staying transparent.

Many packaging teams are finding they can meet performance needs without always choosing foil by default.

The key question is no longer: “Can anything replace foil?”

The better question is: “What material best balances barrier performance, manufacturability, usability, supply chain resilience, and total lifecycle risk?”

These are very different discussions.

Hidden Quality and Manufacturing Challenges

Foil does a great job as a barrier, but it can also hide problems. Pinholes, cracks, or damage during handling can weaken the barrier without any clear signs.

In some cases, finding these problems means extra inspections or tests. On the other hand, some newer clear barrier materials make it easier to spot defects during production and quality checks.

The aim isn’t to remove all risk, since every packaging material has its tradeoffs. The real goal is to know where the risks are and see if they fit with today’s needs.

Looking Beyond Material Costs

One common mistake is judging packaging materials only by their unit cost. The cheapest option on paper might lead to hidden costs in other parts of the system.

When I work with packaging teams, I urge them to look at the whole picture:

  • Supply chain resilience
  • Quality inspection requirements
  • Validation complexity
  • Ease of use in clinical environments
  • Waste reduction opportunities
  • Manufacturing efficiency
  • Risk mitigation

A packaging option that costs a bit more per unit might actually lower overall costs and quality risks over the product’s life. That’s why more organizations are starting to look at total cost of ownership when making packaging choices.

Engineering the Next Generation of High-Barrier Packaging

This doesn’t mean foil is outdated. There will still be times when foil is the best option. But in my experience, the most forward-thinking packaging teams are open to questioning old assumptions instead of treating them as unchangeable.

Medical devices are changing. Regulations are changing. Patient needs are changing. Packaging should change too.

The question isn’t whether foil has served the industry well. It unquestionably has.

The real question is whether choices made years ago still offer the best mix of performance, usability, quality, and risk today. The greatest opportunity for innovation isn’t to adopt something new. It’s being willing to re-evaluate something familiar.
